Shift: Day 5×8-Hour (08:00 – 17:00)
Description:
Job Description: Substance Use Navigator to emergency room and inpatient identified patients. This MSW identifies available resources and assist patients and their families with access to substance and alcohol treatment.
Works with community partners to help patients with mental health disorders get into inpatient psychiatric treatment if on involuntary hold. Connect patients with outpatient mental health services. Participate in committee review hearings as needed to maintain patient’s rights regarding psychiatric holds. Proactively intervenes in suspected abuse/neglect cases. Identify and address social determinates of health. Provide cross coverage for inpatient and birthing center.
Requirements:
– MSW *LCSW NOT REQUIRED, MSW is sufficient*
– 1 year of ED experience, SUN and Case Management experience preferred*
Shift: 5 x 8 M-F Days, 0900-1700 with flexibility to start at 0830, 0900 or 0930 for an 8 hr day
no weekend, no on call requirements
