We are seeking a dynamic and empathetic Patient Engagement Lead to join our healthcare team. The ideal candidate will play a critical role in enhancing patient care by engaging with patients, understanding their unique health profiles, and guiding them toward appropriate healthcare services. This role requires strong communication skills, the ability to build trust, and a patient-first mindset to help individuals make informed decisions about their health and schedule visits with providers.

Key Responsibilities:

  1. Patient Engagement and Outreach:
    • Proactively reach out to patients via phone, email, o consultations to discuss their health needs and concerns.
    • Review and analyze patient profiles, medical history, and care plans to identify gaps in care or recommended services.
    • Build rapport with patients to foster trust and encourage open communication about their health.
  2. Healthcare Services Promotion:
    • Educate patients on available healthcare services, preventive care, and treatments tailored to their specific needs.
    • Clearly explain the benefits of scheduling visits, completing follow-ups, and adhering to recommended care plans.
    • Address patient questions or concerns about services to ensure they feel confident and informed in their decisions.
  3. Scheduling and Coordination:
    • Assist patients in scheduling visits with healthcare providers, ensuring appointments align with their availability and preferences.
    • Follow up with patients to confirm attendance and provide reminders for upcoming appointments.
    • Collaborate with healthcare teams to ensure seamless coordination of care and timely follow-up.

Key Skills:

  • Strong understanding of healthcare services, preventive care, and patient-centric communication.
  • Excellent verbal and written communication skills, with the ability to convey complex information in a clear and compassionate manner.
  • Proficiency in healthcare management software, CRM tools, or other patient engagement platforms. EPIC CRM preferred
  • Exceptional organizational and time-management skills, with the ability to manage multiple priorities effectively.
  • Empathy, patience, and a passion for helping others improve their health.
